CHED Region 10 is now accepting New Applicants for STUDENT ASSISTANCE PROGRAMS for AY 2018-2019

These are the following requirements to submit at the Office of Admissions & Scholarship Administration.

Deadline of Submission is on : April 20, 2018.

I.

FOR COLLEGE LEVEL:

o   Certificate of grades in all subjects in completed semester (from 1st year of admission to latest semester) from the Registrar with Certificate of General Weighted Average per student

 

FOR GRADE 12 LEVEL:

o   Photocopy of Grades in Grade 11 (1st semester & 2nd semester) with Certificate of General Weighted Average (GWA), in percentage, from the Principal (minimum 75% or its equivalent passing grade)

o   Photocopy of Grades in Grade 12 (1st semester) with Certificate of General Weighted Average (GWA), in percentage, from the Principal (minimum 75% or its equivalent passing grade)

 

FOR SENIOR HIGH SCHOOL GRADUATES:

o   Photocopy of Grades in Grade 12 (1st semester and 2nd semester) with Certificate of General Weighted Average (GWA), in percentage, from the Principal (minimum 75% or its equivalent passing grade)

 

II.

Any one of the following:

o   Latest Income Tax Return (ITR) of parents or guardian,

o   Certificate of Tax Exemption from the Bureau of Internal Revenue (BIR)

o   Case Study from Department of Social Welfare and Development (DSWD)

o   Affidavit of No Income

o   Certificate of Indigency from their Barangay

III.
Certificate of good moral character from the last school attended

IV.
Duly filled-out StuFAP Application Form (Secure this form @ Admissions & Scholarship Office)
