REQUIREMENTS TO ENTER MS BIOLOGY, MASTER OF BIOLOGY, Ph.D. BIOLOGY, MS ENVI SCI, Ph.D. ENVI SCI, MS MARINE BIOLOGY AND Ph.D. MARINE SCIENCE

Note: No DOST Scholarship for Ph.D. Envi Sci and Ph.D. Marine Science

A: Department Requirement

• 1 copy of GP-Form 1, original (from Dean’s Office)
• 1 copy of TOR (certified true copy) or evaluation of grades duly signed by the registrar (request slip from Dean’s Office)

B: Dean’s Requirement

• 1 copy of GP-Form 1, original (from Dean’s Office)
• 2 copies of CP-Form 2 (from Dean’s Office and this will be filled-up by your 2 previous Professors)
• 1 photocopy of Letter of Admission from the Program applying from (from Dean’s Office)
• Photocopy of the admission fee receipt

C: DOST Requirement

• 1 copy of DOST Application Form (downloadable from DOST website)
• 1 copy of Medical Certificate (ORIGINAL)
• 1 copy of NBI Clearance (ORIGINAL)
• 1 copy of NSO/PSA Birth Certificate (ORIGINAL)
• 1 copy of TOR (certified true copy) or evaluation of grades duly signed by the registrar (request slip from Dean’s Office)
• 2 copies of endorsement/recommendation letters from 2 previous Professors and the Letter addressed to the DOST Director

Note: Documents submitted in separate Long Brown Envelop with Labels: A, B and C along with your Name, Cell Number and Program to be taken at the back of the envelop. Then Submit all documents to the Department Secretary and e-mail to csm.biosci@g.msuiit.edu.ph
