1. NSO Birth Certificate (2 photocopies)
2. SASE Result (original and photocopy)
3. (4) 1x1 pictures
4. (2) 2x2 pictures
5. Form 138 (original and photocopy)
6. Good moral (original and photocopy)
7. (2) Brown envelopes long
8. Matriculation fee of 4,000
