DETAILS OF JOB
- SCOPE OF WORKS:
- Supply and installation of sliding glass windows, 5mm thick brown glass on 50mm x 100mm tubular analoc frames.
- 13 sets W1: 4-1.00 x 1.20 sliding window
- 7 sets W2: 3-0.80 x 1.20 sliding window
- 2 sets W3: 4-0.80 x 1.20 sliding window
- 3 sets W4: 1-0.80 x 1.20 sliding window
- 6 sets W5: 1-1.20 x 0.60 sliding window
- Supply and installation of 2 sets 0.90mm dia. Fixed brown glass windows, 5mm thick brown glass on 50mm x 100mm tubular analoc frames.
- Supply and installation of swing glass doors, 5mm thick brown glass on 50mm x 100mm tubular analoc frames.
- 1 set D1: 2- 1.00 x 2.10 double swing door
- 11 sets D2: 0.80 x 2.10 swing door
- 1 set D3: 2- 0.70 x 2.10 double swing door
- SPECIFICATIONS:
- Provide air con and exhaust fan openings (refer to the end user the exact location).
- Provide locks on sliding windows.

- TERMS AND CONDITIONS:
- Inspection of the actual site of installation to get measurements and totality of work.
- Secure site inspection certificate from MSU-IIT PPD Office.
- Submit bidder’s bond and subsequently Performance Bond upon award only.
- Submit time duration to complete the project from receipt of Purchase Order in calendar days.
- Have filed Accreditation from BAC Secretariat Office.
B. The following eligibility requirements in three (3) copies , duly certified, must be dropped with your bid proposals in the drop box at the BAC Secretariat Office.
- Letter of intent with the information of the authorized contact person and his/her position;
- Mayor’s Permit for Y-2008;
- BIR Registration with Current Tax Clearance Certificate and Tax Payers Identification;
- Current DTI Business name registration and/or SEC Registration Certificate;
- Bidding statement that the agency is not blacklisted or barred from bidding by the government agencies;
- Bidders letter authorizing BAC or its duly authorized representative to verify any or all of the documents submitted for eligibility check;
- Bid Security equivalent to 1% of then Approved Budget Cost (ABC) or P6,169.00 in the form of check/cash and bidders fee (non-refundable) equivalent to .002 of the approved ABC in the form of cash, or in the amount of P1,234.00
- Articles of Incorporation, Partnership or corporation, whichever is applicable, including amendments thereof; if any;
- Sworn affidavit of the bidder that he/she is not related to the head of the procuring entity, members of the BAC, TWG and Secretariat and members of the PMO, and the designers of the project, by consanguinity or affinity up to the third civil degree;
- Valid venture agreement in case of a joint venture;
- Certification under oath that the documents submitted in satisfaction of the eligibility requirements is an authentic and original copy, or a true copy and faithful reproduction from the original, complete, and that all statements and information provided therein are true and correct;
- Certification of Relevant Experience of undertaking of similar projects for the last two years;
- Certificate of Accreditation issued by the Board of Accountancy (BOA) on the designated CPA as company accountant; and
- Certificate of registration and Accreditation of the agency issued by the Philippine Government Electronic Procurement System (Phil-GEPS).

D. The MSU-IIT reserves the right to review the qualification of a selected bidder, subject to technical and production criteria, reject any or all bids, waive any formality or defect and accept the offer considered most advantageous to MSU-IIT.
